try taking the alternator belt off and see if you can trace it to one of the  moving parts?

I had the head off it and there seemed to be no evidence of valves hitting the pistons, the last time I done the timing it was spot on

does it drive ok? could it be the turbo vanes rattling on the body, take the hose off for a look?
try holding a long stick against various places and putting it to your ear see if you can hear where its coming from

Didn't over heat for long, as soon as I noticed the steam, pulled over onto hard should and stop the engine. Maybe 30-60 secs max .

I have changed the head for an old one and still rattles, new water pump, new thermostat, new belts and new alternator.

Rattle sounds like top end towards the front of the engine, it sound like rattling tappets due to lack of oil.

If I had hair I would be pulling it out  ;D

If you've changed the head it must be bottom end. Could be cam followers. When you had head off did you rotate engine to see if pistons were rocking I had a grudging pin seize causing similar noise but that was on a diesel fiat. I've got a mechanics stethoscope. I could pop over have a look if you want maybe a fresh set of eyes might help.

My 200Tdi developed a light 'clicky' noise which I thought was a loose tappet or cam follower.  Turned out it was the crankshaft bolt had slackened allowing the front pulley to eat away the shaft  :o
